Here is a translation of what they ask:

Nombre = name (first)
Apellido = last name
Direccion = address
Ciudad = city
Estado = state
Codigo Postal = zip code
Pais de origin = Country of Origin = you want to choose ESTADOS UNIDOS
Sexo = sex (male = hombre; female = mujer)
Ano de nacimiento = Year of birth
Estado Civil = ok, don't know direct translation, they want to know if you are married (casado), single (soltero), divorced (divociado) or widowed (viuda)
Cómo escuchaste de nosotros? = How did you hear about us? (just choose an answer from the scroll down options)

Last question = Do you wish to receive.... ANSWER "SI" = YES


Then push green button that says ENVIAR (send)

also on the right hands side there is an english version link to push... it still comes up spanish ...oh well..

If you use Roboform, it iwll automatically fill everything in correctly for you too.

And LSLP, "Estado Civil" would directly translate to "Civil State" or "Civic State".
