My inserts did not have the Tums Quikpak rebate form so I wrote the company.
Here is there response:

Do you think the Tums needs to be purchased between (date and date)? I think I purchased my tums BEFORE the date on the rebate (but I don't have the form to know for sure) so I wonder if I submit my UPC, CRT and the 3X5 card if that'll work. Hm.....

It should work. However you will have to have the correct dates on your receipt. Let me see if I am able to locate the dates for you.
Print out a copy of the above email from Tsquared as a reference when you copy everything for your rebate . (For your own file)
This is great of Glaxo. Lately most companies are simply saying when the forms are gone, they are gone.

I have a rebate form receipt dated 5/18/08 threw 6/30/08, no post office boxes will be honored and be received by 7/15/08
