| Help filling out form... Please help me in filling out the rebate forms.... I have never done one..I am filling it out for the $100 spa certificate where it says qty I assume I put in "1" but where it says Earnings do I put in 1 or 4 because I bought 4 ??? Please help....   |

| When I filled mine out, I put down the dollar amount of the certificates. For instance, for October I wrote "$100" in the earnings blank. Don't know if I did it right,but they claim that the certificates are on their way, so something worked! :-) |
