It was in one of the inserts attached to a $1 coupon. According to the paper, you do not have to send in a form, in fact this is not a form just directions. It states To receive up to an $8.00 cash refund send in your name, address, city, state and zip code. The UPC from Orajel overnight cold sore patch,cold sore brush,Plaque revealing rinse or Plaque revealing pre brush gel. The dated cash register receipt with the item circled, dated after 1-11-09 and postmarked before 4-11-09. Mail to
