To receive your mail-in rebate: Purchase Fresh Sacks and send in this completed form, along with a copy of your original cash register receipt and the original UPC code from the product package. Amount of the rebate is equal to the purchase price, excluding any tax, not to exceed $5.97 US dollars. I don't see an expiration date...

The company that processes their rebates went bankrupt. I had just (finally) received my rebate checks and had them returned by make bank along with fees. I did call Fresh Sacks and was told to forward the information on what I am due along w/ bank fees to:

They don't know yet how many people this is affecting. If you receive your rebate check you might want to verify w/ corporate that it is good before cashing it and having to go through this mess. I will send them the requested information and hopefully get reimbursed. This is the first time I have ever had a problem with a returned refund check.

My rebate check was also returned today with a "stop payment" stamp and a bank fee. Thanks so much for the info on how to follow-up on this situation.

Update: I mailed them copies of the returned refund checks and bank fees. I received a check from Bryce Foster for the amount of the returned refund checks and the bank fees. I also received a box with packages of freshsacks.
