Question Is "Plugged-In People" legit?

I received an email from them stating that I am a potential winner of $100 if I send in the form within 10 days. The email is below wihtout the form that I have to send in which looks standard. Can anyone tell me if they are legit? Has anyone been paid from them? I hesitate to send in my name, address, zip code, and phone number and then find out it is some phishing scheme.

Just for the record, I did get a check and it cleared about 3 weeks ago with no problems. They are a survey company and I apparently won some drawing with them! I rarely get surveys with them, but it's the first drawing I've ever won! yay!
