I am filling out the rebate form and where you have to type in the upc codes there are only 12 lines to write the upc code and i have 16 items worth of upc codes to write down. Should i just attach a separate paper with the additional ones written on that or should i just fill in as many as i can? I am new to this so i'm not really sure what to do!

I don't know the real answer. I wrote the rest on a second piece of paper myself. I would like to know if it was unnecessary though. But I will jump through any hoops I have too.

I printed 2 and filled them both.

I actually called to ask when I filled mine out. I was told I could attach a separate sheet or write on the back. Just make sure to make a note on the front page to see attachment or backside!

I'm so glad I found a copy of this form online still. I think I almost had a heart attack yesterday when I thought I wouldn't be able to send one in after all the purchases I made!
