editing to add, I called Wyeth because the form states "limit two offers per family and/or address" I asked that If I were to buy 2 bottles of Centrum in one transaction ( thinking of CVS ecb deal, ) if I could use that one receipt for my 2 offers. I was told YES!!!, Melanie @ Wyeth said to just send in 2 forms and the 1 receipt (with purchase of 2 bottles) in together and there shouldn't be any problems.

I happened to call them as well and learner that the drive is 128mb capacity. Retail value about $5 or FAR usually.
