Has anyone done this offer? Buy any 4 of these products 4oz or larger, mail in the form, orig CRT and UPC to get a free movie ticket for CARS (usually good for other movies as well!)

They had this advertised at Shaw's, but it can be from anywhere.

Is there a form that you are supposed to use? I have heard rumors of this deal and have been saving my Yogo ups and have all my CRT in a box. Is there a date that is has to be between? Thanks!!

To my knowledge: this can only be done with a form.

I don't know where ppl have been finding the form.
