On the Small & Mighty rebate form it says in the requirements
1) Original Rebate Certificate completed with all the infomration requested including the 10 digit UP number from the product purchased.

However, no where on the form does it have a place to enter the UPC number? Do I just put it any old place? Don't want to get hosed because I filled it out wrong.

I actually just cut it off the bottle and stapled it to the form.HTH
