Requirements:
1. To pre-register your submission prior to mailing go to sirius.onlinerebates.com.
2. Include original or copy of receipt of purchase of radio.
3. Include original UPC code (12-digit #) located on the back of radio packaging.
4. Include original or copy of the ESN/SID# from either the back of radio or radio packaging.
5. Activate by credit card no later than October 11, 2008 with a new primary or multi-receiver subscription.
6. You must maintain 60 days of continuous service before your rebate will be processed.

7. For an additional $20 rebate include original or copy of receipt of purchase of Professional Installation.
8. Mail must be postmarked on or before October 27, 2008.
