To participate in the Corazonas Challenge, one must follow the below guidelines in adopting a heart-healthy lifestyle: incorporate heart-healthy nutrition and exercise recommendations. Consume two 1oz servings of Corazonas heart-healthy chips daily with meals. Take a cholesterol test before and after the challenge to measure results (a total cholesterol test that includes the HDL and LDL breakout is ideal)
If after 28 days participants are not satisfied with their results, Corazonas Foods will reimburse the cost of chips (up to $20), provided original receipts and/or proofs of purchase are included with redemption form. Full details on redemption follow.
Void where prohibited, taxed or restricted. Offer starts 8/24/08 and ends 11/01/08. All claims must be postmarked no later than 10/15/08 and received no later than 11/01/08.
How to Claim: To receive a check for a maximum refund of $20.00, send an original request form (below) and mail it with:
a) The original store-identified cash register receipt dated between 8/24/08 – 9/30/08 with the purchase price circled, and
b) Original doctor’s office paperwork or proof of purchase from a cholesterol screening or testing kit. Actual results of test are not required, only proof of having taken or purchased a cholesterol test between 8/24/08 and 10/15/08. Reproduced proofs of cholesterol test and cash register receipts are prohibited and will not be reimbursed
c) Self-addressed Stamped Envelope with proper return postage
d) Name, address and a brief description of why you were unsatisfied with CORAZONAS
