Just FYI, I called them few weeks ago with request to reinstate the program and put the form back on the internet( and couple of other questions) and as usual representative ask me about my address and e-mail, to send me coupons in the mail(I called them few times before and never got anything good from them) so I gave it to them, and here we go.. they send me few coupons $1.00 yogurt melts and others and THE FORM, so you can try to do it this way.
