Purchase $25 of participating products and receive a $40 cookware set:

I called the 800# this morning and learned YOU DO NOT HAVE TO HAVE THE FORM. Here is the info you need and where to send:


~~Original Register Receipt with participating products circled with price~~
~~Date and store name circled~~
~~List of of the last 5 digits of the UPC codes of the qualifying products.~~


Include your name, mailing address, telephone number. No PO boxes allowed.

Mail receipt, and list of products to:
