I just wanted to say that I called the number today and talked to a lady and she said if I couldn't find a form to just mail everything else to the address she gave me. She told me they needed your name, address, phone number, retailer where you bought the items, original receipt, and the last 5 # off of the UPC codes. She also said you had to buy from a participating store which she went down the list for me. HTH

I have a couple extra forms and I will give you one if you don't mind sending me some coupons. I would love to have Winn Dixie $10 off $50 order or any other store specific coupons. I went and spent my $25 by buying oral b toothbrushes and using bogo coupons I had for those. I also used the coupon for $5 off olay body wash with purchase of a venus embrace razor. Just let me know if you need a form and we can swap info.

I just read the ad starting tomorrow at Save Mart, there's info about the rebate inside. It's said that, "purchase must be made in one transaction". Not good for me because I did the deal in CVS into 2 transactions.

Thanks for the info on this. I'm trying to mail my CVS receipt with the terms & conditions printed from the computer. Can't hurt to try...like someone above said, it's only costing me a stamp!

Toys R Us and Babies R Us is participating. They also have a promo for a $10 g.c. wyb $30 of the same qualifying items, so spend $30, get $10 g.c. + free kitchenware. Sounds like a good deal. For those in Central and Northern Cal that shop at Savemart, their weekly ad reads,

"Purchase $25 of participating P&G products in one transaction and receive a FREE pink kitchenware set from Good Cook's Hope line by mail.

Mail-in offer. If $25 of participating P&G products are purchased, official form will print at check out.
