Ok so clicked on the link to make sure it works and up pops the request form with my name and email address on it, does anyone else see this???? OH NO!

Yep, I did mine and then did one for my mom and I could change everything but email address but it went to next page fine and said her would be mailed to her to?!?

I think it happened because you completed the form before pasting it here for others to use....I have run into this before, so now I cut and paste the form (before I fill it in) on the "new post" window here, then go back and fill the original form in for myself and submit, then come back to the "new post" window and post for others. Hope this helps!
