It's not a rebate-- it's a free Crest Pro Health coupon wyb the same. You have to find the form on a tearpad. I found mine at CVS.
(Unless someone was talking about something else).

I found a form at CVS, too. Yes it is a coupon for the free item, but it functions as a rebate in that you have to send in the UPC code and RR to qualify. I mailed mine off today.

you do not need to send the upc

just save the free coupon for when WAG or RAd has it free after rebate

that'll recoup your cash

hey!! I found one and turned it in, and amonth and a half later it says ''po box closed'' can anyone give me the address on their form??? maybe it's different and I can resend it ???
