I bought the Oral B vitality toothbrush that was on sale at CVS and there was a form attached that you can fill out and send off and they send you a free coupon for crest pro health night paste and one for crest pro health night rinse. Makes the deal even sweeter!! I have an extra form if anyone needs it. I would trade for 2 coke or 2 pepsi codes or 2 pampers codes to the first person that pm's me. All you need to send it in is the form and a receipt for the vitality tooth brush with the purchase price circles. So, if you have the receipt and need the form, let me know.

I bought 2 vitality toothbrushes but only one of them had the form. It was an awesome deal since we also happend to have 2 $3 off coupons (one from the paper, and one from the dentist where we'd gone that same day) plus the $4 off of 20.
